Allocation Questionnaire – If the claim is not settled, a judge must allocate it to either the Small Claims track, Multi-Track or Fast-Track. To help the judge choose the most cost effective case management track, an Allocation Questionnaire must be completed.
The form must be complete and returned with the appropriate court fee within 28 days of filing the defence (no later than the last date of filing).
